Trusting UBM AI + ICLguru vs. Other Methods: Casia2

Background
Devy Mandagi MD delivered a presentation about ICL surgery and the use of Sonomed Escalon UBM AI + ICLguru during a Triastri Speakers Corner event at the IOA 2025 Congress in Bandung, Indonesia on 4 September 2025. Following the 40-minute talk, he performed a live demonstration of a UBM AI exam on one of his ICL candidate patients that he invited to the meeting. He also scanned the patient at the Tomey exhibit booth using the Casia2 OCT. The patient was clearly within the ICL indicated range (e.g., ACD of 3.38 mm).

Approach
ICL sizing results obtained using UBM AI + ICLguru was compared against results obtained using Casia2.
ICL Sizing
The ICLguru report showed that the 12.1 mm size is appropriate. The Casia2 algorithm suggested use of the 12.6 mm size. Based on confidence in UBM AI + ICLguru, including its 41% predicted probability that a 12.6 mm lens would result in a higher than ideal vault (and importantly a 21% chance of hypervault), the 12.1 mm size was implanted.


Results
1-day post-op results showed good agreement with predictive vault and angles remained acceptable (494 µm vault – within 121 µm of predicted value (higher)), confirming appropriate selection of 12.1 mm size.

Conclusion
Based on the predicted values obtained using UBM AI + ICLguru and the actual results obtained from the 12.1 mm lens, implantation of a 12.6 mm size would most likely have resulted in a higher than ideal vault. This example illustrates the benefits of using UBM AI + ICLguru as apposed to Casia2, even for patients with “standard” anatomy.
